Go to http://www.ssa.gov and click on the link that says "Use your ZIP code to find our office" (near the bottom left corner of the screen). You will then need to type in your zip code and they will tell you the address and phone number of your local office.
It is helpful to use the on line service, because some cities have more than one Social Security Office. Your case would be assigned to the Social Security Office that covers your zip code.

Social Security has an 800 number that you can call and get the number you need. I hope this will help you. The number I have is 1-800-772-1213........Good Luck!....You can also contact them on line by going to http://ssa-custhelp.ssa.gov
