it all depends on your position. if your are a server/ waitress, the restaurant would probably have a standard uniform they would give you or you would have to purchase. if you are a hostess, some restaurants might have restrictions on what you can wear, but as long as you look neat and your clothes are not too revealing (unless you work for Hooters) you should be fine. if you are a manager, dress professionally, business like. if you work in the kitchen, the restaurant would usually provide the uniform.
just wear black pants, long sleeved shirt (preferably solid color, white or sometimes black if you are a server), closed-toe shoes that are comfortable and flat-heeled. don't wear a lot of jewelry, since most of the respectful restaurants would have restrictions on the amount of jewelry you can wear. some of the restaurants even have the restrictions on the amount of make-up and perfume you use.

Depends on the restaurant. Chain etc. But black slacks and a white blouse is a good idea if not given further instructions then ask what is appropiate before you leave on your 1st day.
If a guy, white T-shirt black pants.
