You might want to download the manual for the Meade DS-xxx series of refracting telescopes. Assembly instructions are in there. Just reading that will give you a better understanding of how the thing works.

There should be a set screw to hold the eyepiece in the eyepiece holder (a little thumbscrew that goes into a threaded hole in the tube the eyepiece slips into). Loosen it enough to get the eyepiece in there and then tighten it to hold it in place. If the screw has fallen out and is missing, you will need to get a replacement. You will have to go with trial and error to figure out what size screw you need (it does not have to be a thumbscrew, of course, any kind of cap will do, but a thumbscrew is easiest to tighten reasonably without any tools).

If trial and error does not appeal, contact Meade directly.

All refractors flip the image upside down. You will need to get a special eyepiece type optic called an image rectifier to re-reverse the image. Usually your eyepiece will mount into it, and it in turn will mount to the telescope.

It sounds like you might need to purchase a new eyepiece holder. That sqaure of glass should be reflective. Sounds like when whatever cement was used to mount the thing came loose, most of the metal coating was left behind.
