Both prostate specific antigen (PSA - a blood test) and digital rectal examination (DRE) should be offered annually, beginning at age 50 years, to men who have at least a 10-year life expectancy, and to younger men who are at high risk. (strong family history of prostate CA) Information should be provided to patients regarding potential risks and benefits of intervention.
It should not be painful. A single index finger with copious lubrication is inserted. Men do not seem to find it terribly uncomfortable - perhaps cold from the lubrication.

It depends on family history, or if you have any gastrointestinal problems. Generally age 50. I am 30, and have Crohn's Disease and have regular prostate examinations every 6 months. It is not painful, but can be a little uncomfortable.

The American Cancer Society recommends a prostate exam for all men aged 50 and up. Men at particularly high risk of prostate cancer should start being examined at age 40. It is uncomfortable, but not painful.

Forty might be a good time to ask your Dr. My husband said they do not hurt the body just embarrassing to the ego. Regardless, better to have a little discomfort that letting the problem develop. That will hurt FAR more.
