As with heterosexual couples (man/woman) they can either have their own child, adopt, or invetro-fertilization.
Same sex couples usually have a opposite sexed friend (some may be gay, some may not) that go into an agreement to "conceive" a child and let the child be raised in the same sex relationship. I have seen this alot.
But usually, it's the same guidelines for any type of relationship when it comes to adoption.

For men, the choices are adoption and surrogacy. For women, the choices are adoption or sperm donation. Adoption is the more ethical choice, and the most common, because lots of children that already exist need families and because overpopulation is becoming a big problem.
But gays and lesbians can definitely have families. They aren't any different than a couple where one partner is sterile.

They can adopt, opt to be artificially inseminated or ask a prospective father to naturally impregnate one of them (obviously, if its a lesbian couple). I have been asked to do this before, but turned it down.
