An affidavit is simply a statement that is taken under oath, sworn statement. There is no blanket form for this. You can type up your statement, take it to a notary who will then accept that you are indeed the person who is making and signing the statement. In courts of law, judgements do not need any support at all since they are final.

Yes there are. You can purchase a book of such forms at most bookstores. I suggest Barnes & Noble because they're always in stock there.
