It means that you have achieved less than what could be reasonably expected of you, taking into account your intelligence, etc.

An underachiever is one who achieves less than his/her natural ability and intelligence. Many young, bright people underachieve for a variety of reasons. Very intelligent students do not reach expectations mainly because they are bored with much of the routine assignments offered in and out the classroom. For example, they cannot understand and do not accept that if they know how to factor in algebra after doing five practice problems, why they have to do 50 just because they were assigned. Resentment leads to rebellion in the form of just not doing such mundane assignments.

I believe that the word underachiever refers to the lack of motivation and initiative of an individual. Then again, we cannot truly measure a person's drive by comparing with others. We've all heard the expression: "They won't apply themselves..." As learners, we are diverse in the way we look at a problem and its solution. Someone may not be motivated at work; then again, the work may be boring to that person. Same way with school aged children. It is not the lack of intelligence, but perhaps the way things are presented.
