That all depends on what you agreed to and what the court ruled. I am dealing with work comp and my lawyer told me that he would not collect anything until the case was settled. However the court ruled that 1 check out of every 6 would go to the lawyer for his expenses. But he let me have it . So it all depends on what you agreed to or what the court ruled.

There is no such thing as standard. If you ever question a rate, shop around in your local area. My guess is that you already have a contract with your lawyer and the questions is moot.

That would be according to the agreement you had with lawyer when you hired him to help you collect the workers comp

yes, I worked for a lawyer for 10 years and this is what he took (even though most of the time he didn't take it because it was the client's paycheck)
