Actually this may be quite difficult if they are in any sort of institution because their privacy will probably be protected on medical grounds. If they are literally roaming the streets then perhaps various homeless charities might be able to help... although again you might run up against problems of confidentiality.
Have you considered hiring a private investigator? One who is a local ex-cop might have enough contacts to help you out.

I don't know about southern CA, but back east; go to NYC and stand still - one will pass by in a few minutes
