I will give you a broad sense of this case (as I have no idea about Indiana) to guide you:

Some jobs require a person to have a minimum weight in order to yield an expected performance. The best example is a prison guard. Previously, they had a gender requirement (women couldn't be prison guards) but that rule was shot down in court and replaced with a height and weight requirement - as it was the guards ability to subdue rowdy prisoners more than anything (in a woman-only prison; prisoner-on-guard harassment excluded).

So, I would say that, as long as the company can argue that a certain height and weight is a requirement of the job, they have the right to establish that on the app.

Yes and no. It is illegal to hire someone based on their height and/or weight, however if the job environment would be hindering to someone to large or tall the employer could ask for the information to see if the person would be able to do the job.

Would your weight and height have anything to do with the job? If not, then it's illegal.

I don't know about that state, but unless it is for a job, (like some jobs, you have to be under a certain weight and height, like airline strewdress, maybe) it is wrong. They shouldn't be askng that.
