There are a variety of ways, depending on how the guestbook is setup. Various exploits can be used, some of them include SQL Injection into the database, various exploits in the programming language it was coded in, down to exploiting the web server it's hosted on and modifying the database from there. These are only some of the ways someone might go about exploiting that.

A determined hacker can hack anything, no matter how much security you put in place. The question you should be asking is "will anyone want to hack my _guestbook_?" to which the answer is most likely "no, there's nothing to be gained by doing so - it's only a freaking _guestbook_, hardly a major NASA server...".
