No. Mass. has gay marriage on the books. Connecticut has civil unions which are somewhat close. Hawai'i has a domestic partnership law (I believe).
There are 45 states that have either laws or constitutional amendments defining marriage as between a man and a woman. In some of those states, there is room for civil unions or domestic partnerships. In some, they are disallowed.
Yes, Vermont also has a civil unions law as well.

So far as I know, Massachusettes is the only one to actively perform them. That having been said, it's not explicitly banned in all other states. Many will accept out of state or even out of country same-sex marriages.

However, very few states completely allow gay marriage... others compromise with 'civil partnerships', not quite marriage, but somewhat of a similar concept.
