This is a male 63 years old, does not seem to have any problems with urination. No blood in the urine. States that it seems like the left testicle will swell at least 3 larger than the right testicle. Then it will go down a little bit. Did try an antiobiotic, did not make any difference. Does have pain-in the front part of his legs, but has severe hip pain, so not sure if this has anything to do with the testicles swelling.
Is not crazy about going to a doctor.

This is probably an inguinal hernia given the age. I would be interested to know if he remembers when it happened and what he was doing at the time (lifting a heavy object). The treatment is surgery, likely laporoscopically, to reduce the hernia and fix the defect. The defect is an open processus vaginalis through which a portion of his bowel is going through. He needs to go see a doctor because it could get strangulated (meaning it will lose blood supply). If it does, the mortality rate is 8-9X more than if he had it repaired on an elective basis.

Other possibilities are testicular torsion and a hydrocele, but these are uncommon at his age.

It could be loads of things, from hernia to pinched nerves, going to the doctor is the only way to find out what is really going wrong with your health. Hips pain can also be associated with leukemia, perhaps your doctor will want you to have a bone narrow test, by taking a small piece of hip bone. It's only a 15 minutes procedure and it hurts as little as an injection.

The best of luck and hopefully there is nothing serious going on.

Be well and take care.

given the size I would agree with others - sounds like an inguinal hernia

it is possible he is getting a torsion or severe epidymis infection but to swell to three times the size of the other is quite a feat

I would seek medical advice asap

a scan should resolve the problem
