But if you just want to get fast relief they recommend you alternate between hot and cold. I would say 10-15 minutes of each. Be careful not to scold or freeze yourself. The cold should not be directly applied. Have a towel or two wrapped around it.

Neither, gout is a systemic metabolic disease, caused by elevated uric acid, thus you need to lower your uric acid to have pain relief, all others are palliative treatments. Its better to get prescription for the strongest pain relief drugs. There are drugs during gout attacks and another drug to prevent attacks. Consult your doctor about this. Moreover, check your diet, you might be taking in a lot of high purine foods.

Use cold packs, if you have gout you should lower your intake of red meats, and if you are a drinker you should not be consuming any alcohol.drink at least a gallon of water per day.
