Go to the start menu, then select "RUN". Type in DXDIAG. After a littel bit, a window will appear with information about your computer. Click on the "Display" tab at the top and it will tell you what model of graphics card you have installed.

Physically removing your video card from your system HAS to be done correctly.
But it is simple. make sure that you PC is switched off AND the power lead unplugged.
BEFORE attempting to take your card out earth yourself out on the chassis of the case (bare metal). If you don't do this you run the risk of BLOWING your card.
If you are removing a modern card, AGP or PCi Express card there maybe a clip at the end nearest to the middle of the board, you will need to move that across as you take the card out.
ANOTHER WAY OF FINDING OUT what your card is . is by using software like Driver Detect or SISoft Sandra. Just type either of these names into Google and you'll find plenty of download sites.

If you have a DELL, Gateway, HP or Compaq, use their website, and search for drivers for your model computer. If they show several models, GET THEM ALL, and try them.

IF you yank the card, where down the brand name of the board, and brand and number of the big chip. So you might find a PNY board with nVidia chip.

Then join http://www.driverguide.com and look for the manufacturer through there.
