I think that one possible explanation is the frequency with which doctors interact with people. They like to project a trustworthy and qualified image since they are dealing with peoples' health. Engineers don't necessarily need to cite their level of education as a way to assure people of their work. It is not a discrepancy in how hard one works for their title.

The word or title doctor is mostly used for medical practitioners. Other doctors are those who have done their doctorate degree. As it is commonly used for medical practitioners only it refers to respect the society gives to these professionals. Also they can be called at any time of emergency. Whereas, Er. for engineers sounds superfluous. It will only refer to a degree or qualification attained which is definitely useful to the society but will not be seen at the same level.

Well you are thinking that only medical doctor's are doctors, there are many disciplines that a doctoral degree can be attained, Math, many sciences, studies in law, and many human and animal behaviors. However it is customary that only People that receive a PhD. In medical or education use the pretext of Dr.

The word Doctor can be used as a title. It is gained by doing a lot of hard work. The word engineer isn't really used as a title that often.

I checked my county's web page. Our County Engineer has P.E. and P.S after his name. I know that P.E. stands for Professional Engineer. P.S. I think stands for Professional Surveyor.

An Engineer has a B.S. in Engineering and most likely a M.S. (Master's of Science) in Engineering. So, those initials will FOLLOW his/her name rather than PRECEDE it.

A Dr. (M.D. or PhD) has a lot more education, so to speak. The M.D.s and PhDs go on for more education (several years, in fact) than the Engineer.

Yes, I have seen lots of people put Er. before their names.

But it looks kinda stupid to me..

Doctors is a level of education.
like a doctor of Music or art.

because it would sound stupid to introduce yourself as Er. John Smith. People would think you had to concentrate really hard to remember your name... lol
