A tattoo parlor, but make sure its a clean one, cause a lot of the trashy ones you can get infections cause they don't clean the needles well.. But you will need your birth certificate, picture ID, and a parent or I think someone over 18 or 21 with you. But make sure the place is very clean.

no. Just wait til you are 16. If your parents won't sign off on it, then that probably means that they would be upset anyways if you went behind their backs.

Licensed I don't know praly no one, buttt unlicensed you could find someone to do it for you for cheap, but i'm sure you wouldn't think it's safe.
