It is Slang for a very low gear ratio, used for maximum pulling power. Today, we call it "Low Range" my Jeep has 4 wheel drive. With 10 gears forward and 2 reverse, 1st gear in low range is called Granny...It just Goes SO Slow...., But, Granny doesn't go fast but pulls like a Dentist on Steroids.

Grumpy is the only one that's got it right. Granny gear is an underdrive. Usually about 1.2 ratio below main drive. They sell underdrive kits tha will give you a granny. They are for dragsters. Tailshaft mounted.

It is like the opposite of overdrive. A very low and slow gear usually used in trucks to get up a steep incline with a heavy load.

Typically refers to 1st gear on a manual transmission.
The reason is because it's so easy to take off in and much slower... some others prefer to use 2nd as their take-off gear instead.

JD is correct except it was a term used in older trucks and pickups. 1st gear was exceptionally low just for the torque it could produce for pulling. It was so low it was not even used for driving. Pickups were for working, back in those days.
