First that depends on what you did, and what your GIRLFRIEND will accept as an apology (if indeed she WILL accept one).
However, when dealing with situations as these, HUMILITY is always the best way to go.

well this girl must be important to you because you really want her to forgive you, so what you should do is take her aside (alone) and say how sorry you are and never do it again or try even. Tell her how her not speaking to you has been on your mind for a long time. tell her she is important to you and you need her to speak 2 u again

It depends on what you did.. it depends on the severity. If you missed a date, then an apology and maybe a gift will make up for the loss and wasted time. If you cheated, or did worse.. then there's going to be more sympathetic apologies. It all depends on the situation, and the girl.

Don't just say you are sorry. Tell her you understand why the thing you did to get her mad was wrong, be specific, and swear you will never, ever do it again. And keep your word!
Flowers might also help.
