If you have hit some real bad holes in the road then you probably have an alignment problem causing it to go back and forth, take it to an alignment place and tell them what it seems to do and they will check it and see if something is loose or there is a alignment problem

This is a common sign of a bad tire or bent wheel if it happens at low speeds. If you want to check for this, replace one rear wheel with your spare and take a short drive at the speed you notice the problem. If the problem gets better or goes away, you found a bad wheel or tire. If not, try the same thing on the other rear wheel. If it happens at higher speeds, is could be an unbalanced wheel and tire.

OEM shock absorbers have a life expectancy of about 25-30K miles. Since you're well past that, I'd suggest replacing your rear shocks first. Then if the problem still exists, look at bushings and rear axle attachment points.
