It sounds like you're being very safe. First, if you used a condom then there's really no chance of pregnancy. Second, if you are on the pill, you are very protected (unless, as someone said, you're on antibiotics or you threw up when you took your pill). Finally, sperm only lives for 3 days. If you forgot your pill on the 4th day after sex, then the chances that any of the spermies are still kicking it up there are slim to none, plus your egg would've had to form, pop out and get near the sperm to complete the conception process.
So the long and the short of it is - if you do wind up pregnant, then you're probably carrying the chosen one because that would be a miraculous conception.

No it doesn't rule anything out. Woman mistakenly believe that if they bleed they must not be pregnant. The fact is that woman can continue to have periods throughout the early months of their pregnancy. Now it may not be a full bleed, it may just be mild, but so many woman are on birth control pills these days that limit the bleeding so much anyways that they can't really tell. Chances are taking the pill two hours late won't cause pregnancy.. but there is a failure rate with all birth control pills, that is why if you really don't want to get pregnant we suggest using double birth control practices while being on the pill.

No reason to worry. If you always took the pill in time (two hours difference should not matter) and never had diarrhea, vomiting or antibiotics (all may put the birth control pill out of function!) it's all OK.

If you used a condom in the first place, the risk of a pregnancy is quite low. The bleeding could have other causes (I don't think it's your period, I guess you'd know when that comes). It's for the best to see an obstetrician, however.

Always get a pregnancy test when in doubt. My sister had a period for the first 4 months of her first pregnancy, so you never know until you take the test.

I don't think it can rule out anything. You should go get checked. Bleeding at any time besides your period is a sign that something is wrong.
