This is one that most people think they know, but are usually mistaken. The well-known "grass skirt and coconut bra" costume that most people associate with Hawai`i is actually not a traditional Hawaiian costume.

In ancient times, Hawaiians typically wore garments of kapa cloth (barkcloth)...women wore a wrapped skirt (pa`u) with bare breasts and typically wore a loincloth (malo). Today, many hula dancers that dance ancient hula will wear pa`u skirts/tops or malo made of modern fabric; however, some do still use kapa.

After western contact, western fashions worked their way into costuming (due to the pressures of Christian missionaries) and the holoku and mu`umu`u evolved. The famous "aloha shirt" evolved in the 1930s as a result of the plantations and the mix of many cultures moving to the islands as a result.

So how in the heck did the grass skirt come from? It's actually an import from Micronesia in the early 19th century. The "grass skirt" that you see serious hula dancers wearing today are typically made from ti-leaves.

There are many cultural costumes of Hawaii and since you are asking, I am going to assume that you would like to know about the original costume now worn by dancers and others for special events.
This consists of a "grass" skirt ( originally made from fronds) and a coconut bra or top, and the Lei. Of course you can go barefoot or wear toe sandals. Also a head dress can be worn which is usually a circle of leaves, but now more often is cloth or flowers. Most polynesians wear leaves tied around thier ankles also..
Of course the costumes have evolved over time and are now represented in may different ways. The missionaries, when they came, demanded that the coconuts be replaced with a more modest bodice and so now you can find the bodice as a part of the costume..it can be fancy (beaded) or plain.
You will always find flowers as a part of the womans costume as they are symbolic.
While it doesnt deal specifically with your question you can see an example of the basics at the link I have provided..

Oh you don't know? I think it is the most famous and well-known costume in the world. Since childhood i'm familiar with it because we danced as hula dancers like PEARLY SHELLS or TINY BUBBLES.
