Which country and from where? I assume from India to US. Now here is what I can say:
There is no prerequirements other than you have to go to an HDFC bank and pay the fees for the visa. However they can reject you if you cannot prove to the visa officer that you have full intention to return back. It is your duty to document evidence that you have the financial capabily to carry-on the travel and have reasons to return back. Here are the tips:
1. If the traveller have money in Indian banks or properties in India, document those and show those in the application
2. If the visitor has a sone or daughter in US then create a letter to visa officer from son/daughter saying th that they have invited the visitor.
3. It is also benifitial to fill up and notorarize I-134 Affidavit of support by the relative in US
4. Have specific answers for the visa officer... Always say that you need to return back within 3 months or so.
5. Some people visit US to help their relatives during child-birth or to look after young children. Never state that as reason. According to INS it is kind of illegal work and visa will be rejected immediately.

There are some main factor for regarding Visa
1. You need to have a valid passport to apply for a visa to any country.
2. You should not have any criminal records.
3. You should have a purpose for the visit.
4. Enough Money in your account for applying for visa.

1. Evidence of sufficient fund to support yourself and any dependants while in the country,
2. Evidence that you will not overstay, in other words, evidence that you will come back to your home country (eg: job, investments, etc.)
3. If you have an invitation from someone or an organization, will help too.
From my experience, those are the basic requirements.
