I would have to say "The Texas Chainsaw Masacre", hands down!
(I am talking about the "original" one here)
I was at a drive in with a girlfriend and we thought we'd be "all tough" and go see the double feature. We neither one were too keen on horror flics!
This drive in had the old speakers that attached to the car window.
Shortly after the movie started, we were seeing the credits and noticed that this story was "true". OMG! We were both freaking and decided to wimp out and leave.
Hearts racing and wanting the hell out of there, we pulled out and I sped out. Can you see where I'm going with this story???
Once we were about a half of a mile down the road, we realized we forgot to take the speakers and set them back into their holders. There they were, hanging out the window, wires and all.
We were never more scared OR laughed so hard in the same night.
I often regret not keeping the speaker (I did for a long time as a memory) b/c now, the drive in is gone and those speakers are considered rare/valued items.
The new version (out a couple years ago) I went and saw with a b/f at the time (in the theatre). Once again, I'm all freaked out! (Will I ever learn? LOL) I couldn't get that monster's face out of my head. Anyway . . .
Next morning, I'm walking my dog up by the river (near my home) and there is a woods right across the way. It was very early morning, so it was dark. I was still thinking about the movie when all of a sudden I hear this chainsaw starting up! (I am NOT joking - this REALLY happened!) Turns out that it WAS a chainsaw, but from the factory across the river. OMG! I was so scared that my sugar dropped! I started to drag my dog while running, forgetting that I might have been choking her!
I shared this story with my b/f and couldn't stop laughing at me! NOW, I can laugh, too. And it's a fun story for me to tell!
I hope my answer was a good one for you!

The Exorcist is the most visually scary movie but for real horror the original movie The Haunted still makes me keep the lights on.
There is another one made in the 60's and it scared me pretty bad but i cant remember the name right now. The characters were creepy and the ghosts were even creepier...

Mmm I though that movie was on the weak side as far as being scary.
Check out "In The Mouth of Maddness" that was pretty scary.
Its about a horror writer that disappears and the publishing company sends a rep and a private investigator to find him.

The Exorcist...there was a really bad fog out the night we saw this movie and that sort of set the mood for the rest of the night. Today I still can't stand the song Tubular Bells (I'm talking Original version here).
That movie was the one that weened me from horrors...but my kids now watch horrors all the time. Ewwwww :)

The scariest/ creepiest first and last scenes I've ever seen were in "When a Stranger Calls." I still can't even think about that movie when I'm home alone. And for horror: "Candyman" is actually really scary.

For me, it's the original "Poltergeist." I saw it when I was 12, and I couldn't eat steak or open my bedroom closet door for weeks. Oh, and while I didn't bat an eyelash when our kitchen chairs would inexplicably stack themselves BEFORE "Poltergeist" came out, after I saw the film I started asking a few questions about it.
("Poltergeist" also helped fuel my lifelong love of horror movies, so there's that.)

I am a horror movie junkie so it takes a lot to scare me. "Ghost Ship" was one of the creepiest movies I've seen. Also "Session 9" which takes place in a real abandoned mental institution.
