I think they keep the baby's last name of the baby's parents. Then when the baby is adapoted the new parents can change the baby's name to theirs.

normally the careholder of the child that is giving the orphanage the child has to fill out papers... all they know of the family background... if they agree to let the child look them up and contact them in the future... so normally they would already know the last name to use. if noone knows the last name, if the child gets adopted, they take the last name of the family, if not, then the orphanage comes up with the name or the child does as they become of age to change their name.

all babies are given their mother's last name. if the mother immediately relinquishes her parental rights, i believe the foster parents or the case worker for the state have the right to chose that child's first name. after adoption, the adoptive parents can change both first and last names if they so wish. i don't think there are even orphanages that are still operated in the united states. i think everything here is done through the state health and human services agencies and foster parents.

usually the last name of th mother, since it is on the birth cirtificate and hospital records. However, very few babies actually go to orphanages, since there is a large demand for infants among the people unable to have children.

That's a good question... I saw a movie once where the older sister adopted all of her little siblings and the judge let them pickj there own last name...they chose Weathery, since they'd through so much nbad weather
