Depends on what the plug looks like.
If it has the big 4-pin connector (though the fan probably only uses 2) then the power supply. If it has the smaller connector then the motherboard, unless your power supply has the matching connector. Really it doesn't matter. The motherboard normally has two ports for fans... one marked sys fan and one marked cpu fan. If you plug into sys fan it lets bios monitor the fan... but really it doesn't make a difference.

Either. Depends on the motherboard and the fan. If you want visibility of the fan in BIOS then you don't want to plug it into the power supply.
As long as you have the correct power connection it really does not matter.

The case fan plugs in the motherboard, but the power has a fan itself that is inside it & plugged in there, but as you may know, power is connected to the motherboard itself; so all fans are plugged in motherboard.

The power supply (the entire piece) attaches to your case. You plug the power cord to the power supply, not the case. The power supply is connected to the motherboard.
