I did a search and this is what I came up with, it is not exactly what you are looking for but it will get you started.

The criteria I used were these:
worksheet on skill to work
Without any quotation marks. There are more websites and data if you put in that criteria. Good luck on your search.

Also, in lieu of a survey, you might just think about which characteristics make a good school counselor and see which of those characteristics you (or others) see in yourself.
