A person's Intelligence Quotient is his/ her ability to cope effectively with the demands of his/ her immediate environment, the ability to assist anybody to think rationally, and interact with others in the best possible way. On the whole it assesses one's general cognitive, conative and affective aspects in relation to one's environment and surroundings.
IQ Score depend on the adaptive behaviour of a person which can be altered to a certain extent only in the early years of life through extensive real life experiences. But, after the age of 18 years and above one cannot see significant IQ score increments as the brain reaches its maximum level of development. We as human beings would never be able to achieve a perfect IQ Score as there cannot be any such perfect IQ Score.

If by perfect you mean getting 100% of the answers right, then yes - it would be possible. It is also possible to "cheat" on IQ tests - they are made up of a fairly standard arrangement and you an actually "learn" to be good at them specifically without actually understanding or knowing as much as the end result tells you.

But as others have said, they are not scored like a regular test.

IQ scores are an artifact of how a given test measures what we define as intelligence, no tests are perfect, they all have errors of measurement and an IQ score is best thought of as a range of scores i.e. you score 115 on an IQ test your "true" score would fall in a range of 110-120. Therefore there is no such thing as a perfect IQ score.
