This is usually asked to put a dividing line in the spectrum of sexual activities, i.e. crossing the sex line. It usually starts getting controversial at one of the following: fondling breasts or genitals over clothes, with skin to skin contact, oral sex or anal sex, or the uncontroversial "penis in the vagina" intercourse.
What I've noticed is that people put the line where it is convenient, but then once they've made that call they are stuck with that line.
Often women will say it doesn't count unless it is intercourse, or if they are trying to slow things down they may say it is sex at the point that they aren't comfortable.
Most men that I know will be happy to clarify exactly what they did if there is any confusion.
"Did you have sex?"
"No, but I got an awesome bj."
