If a character is required to do nudity, they may choose to have a body double. They have g-string like underwear that they can wear to cover their genitals; but usually the skin that you see is bare skin.

some actually reveal their bodies like hallie berry in monsters ball.. and some wear the body colored clothing.

Some do, but I think most wear something and with good lighting/strategic placing, you can't see they're actually covered.
