Oh, yes. Ballantine books released a series of fantasy paperbacks in the late '60s and '70s, and there were several James Branch Cabell titles. I read as many of them as I could find, including Jurgen and Cream of the Jest. Other books by Cabell in the series were Domnei, Figures of Earth, The High Place, The Silver Stallion, and Something About Eve.

Yep got Cream of the Jest right in front of me first printing paperback 1971, intro by Lin Carter, Ballantine Books. Part of your question about knowing about Cabell in the US on the first page it says he was a giant of American letters and ''there is not in fact, anyone on either side of the Atlantic who can touch Cabell when it comes to highly sopisticated, polished and elegantly bawdy comment on the human condition''.Now that I got it out I just may read it again, excellent read.
