If it is family, just tell them you don't want to be called the bad name. As for the rest of the world, introduce yourself as the name you do like.

I went through the same thing with my family. Some of them called me by my first name which I like, but others insisted on calling me by my middle name which I hate. So I just asked them to stop this way they knew I did not like that name and then after that I ignored anyone who called me by my middle name. It takes a little while but they will get used to it and call you the name you like.

This is one of those things you are going to have to live with.
I'm guessing the name you hate is the one you grew up with. I too am that way. I grew up with a certain name that I absolutely hate. However I have realized that they grew up calling me that, so it would take a long time for others to change thier ways if they ever do.
My only suggestion to you if you can not live with it is keep trying to inforce the name you like. In other words when somebody calls you the other name correct them.

c. in my experience, people will call you whatever they prefer calling you--even if you specifically tell them you'd rather be called by another name!!

ask nice, for them to use the one you like...
and just don't answer to the one you don't...
you will eventually re-condition them...
