I usually unzip files in an empty folder, such as c:\tmp (if it doesn't exist on your machine, create it; if it's there but not empty, try a different folder name, such as c:\temp until you have an empty folder).

Let's say you have Winzip. If you right-click the ZIP file, select Extract to: and select c:\tmp (or temp or whatever). Don't forget to also select Use folder names to reproduce the structure of the original files. Once you've unzipped the ZIP file, cd to c:\tmp and look for a MSI or setup.exe. Double-click that file to start the installation.

Also make sure you do not have any later versions of any Visual tool as setup can have problems figuring out latter installs.

It's been a long time since I've installed VS2003, so you may run into other issues. If you do run into problems, VS setup will usually save a log file somewhere in the setup tree. Just look for *.LOG and it will give you some answers (if you've seen a bazillion of them before, LOL).
