If your turtle is lethargic, does not feed, does not bask or constantly basking, swims poorly (i.e. lop-sided, drifting), and/or runny nose (when out of water); usually, it is a combination of these symptoms. If you have only, it may not indicate it is sick. But generally, you know if your turtle is sick when it is lethargic and acting differently.

When you do suspect the turtle is sick, raise the water temp to 80F and basking temps to over 95F to strengthen its immune system. If you have a good herp vet nearby, bring it ASAP for immediate treatment. However, sometimes, certain hatchlings aer never intended by mother nature to survive, even with the best care.

If they do not eat, obviously, there is an issue. If the poop has a strange odor, color, or consistency, or if they don't poop at all. If the animal seems to be weezing or breathing irregularly. If the shell seems to be strikingly different in color. If the animal does not hardly at all come out of it's shell. Mostly just things that are out of the ordinary.

find a herp/reptile vet and get antibiotics quick

Most probably if it's just sitting there with its eyes closed...
