First of all, you may want to consider a larger cage, for the sake of all the lizards.

Second, you will want to make sure that the lizards are eating. It is possible that the three active lizards are eating all the food before the last one can get any. You should consider feeding it with an eyedropper or similar liquid food until the at risk lizard is more active. Although your lizard is carnivorous, if it is having trouble eating, you could try a mixture of foods until it is better.

Finally, make sure that you have adequate heat lamps for your lizards. Almost all lizards require UV lamps to bask under to help with their digestion. If the lizards are not getting that heat, they are not getting the most from their food, and might even have a bowel impaction.

If you don't see some improvement after a few days, you should at least call your vet, if not bring your lizard in. Remember, though, that if your lizard is actually sick (wiht a disease) that your other three might catch it too. If you can get a new cage, and can provide heat to two cages, then you might consider moving the healthier three to the new cage until the fourth one is feeling better, in case the at risk one has something contagious.

I think you should take it to the vet. Lizards are real finicky, and if their environment isn't just right, they'll perish. Is the enclosure warm enough? Do they have enough food and water? And is it the right kind? Do some research on the internet and see what type of environment they're most comfortable in.
