In the day, prior to the advent of Summer's Eve, mom and grandma took care of their personal hygiene via a rubber water bag that suspended from the bathroom door, with a hose and nozzle protruding from the bottom of the bag. The proper spelling is douche bag, douche being French for "wash" or "shower".
To this day the debate continues on the merits of this practice. In any case, I believe in your context this would be an insult of great magnitude.

It's something for feminine hygiene. If someone called you or someone else a douche bag, it's an insult meaning jerk.

Well ...have you ever tried to get the fishy smell out of fish? What do you do? You dip the fish in milk.
That's the purpose of a doosh-bag ... it gets rid of that fishy smell.
