This question can be answered properly only if you provide the place and duration of your visit. Also, "third-world country" is not a hard-and-fast definition.
Generally, most "third-world" countries are safe to travel to if you are properly prepared. In terms of food, it is safe to eat fully cooked foods that have not been cross-contaminated with raw or ditry food and utensils. Eat fruit that has been PEELED.
It is not practical to be visiting countries and hauling your own food if you are staying for any length of time. Best to learn the particulars of the place you are staying and prepare properly . . . more, at least, than checking in with Yahoo Answers! and relying on general information! ;)
However, if you're in a pinch and are hoving off for Congo in a few minutes, stick to oranges, and you'll be a-okay!
Water: Bottled water's biggest market is in 3rd world nations. This is not a coincidence. Bring along a First Alert water purifier (takes out things even as small as viruses), and buy bottled water, sodas, and canned drinks. Do NOT drink tap water or even mildly boiled water. Even in strange First-World nations, you can easily get sick by drinking the water. Your system just isn't localized.

Third world countries are very food oriented. Usually, if you go there a town member will invite you to their place and feed you ... and you will be insulting them if you don't eat the food they prepare for you. Even if the food looks like its off a "Reality TV show like Survivor" ... well, be polite, taste, and hide your discontent.

If you're a picky eater. Otherwise, just be careful what you drink. And don't eat salads as they will ultimately have been washed with the water you're trying to avoid. When I travel to poorer countries, I even use bottled water to brush my teeth.
