Spoken appreciatively, it would suggest a wholeness, connectedness, and balance in your life. In harmony with nature, wise beyond your years, loving of other people. Seen and appreciated as a mother figure by many, not just children or not just your own children.
When used negatively, it brings to mind hippy-dippy utopian ideals, furry armpits and communal living while raising semi-feral children.

It depends who says it and the context in which it is being said.
They might just mean that you are a good mother as Mother Nature aka Earth Mother is a good mother to all of us.
I don't know ask the person who called you that.

It means you are a back to basics kind of person in tune with the earth,doing no harm or pollution and giving something back. You always wear an apron and bake nice sugar cookies.

It means you are like Ceres, Demeter, Eartha or Gaia. That is, fruitful, fertile, abundant (in affection), stable, well-grounded, well adapted to natural currents, fond of investing in things (and people), and, of course, a good mother.
