Dr. Phil would cut to a commercial to hide the fact they just started brawling. And since he's such a big, tall Oaf, Oprah would be reduced to waddling up a large ladder (with a hammer in hand), aiming her strength at the apex of Phil's shiny big vulnerable head.
Phil would promise the audience that if he overcomes Oprah everyone will receive a free copy each, of one of his newest psychobabble books, as a free promotion for himself.
Meanwhile Oprah's three-layers of makeup would be running down her face, as she cried, threatening to pull the funding from all Harpo company shows (which includes good ole Doc Phil's too).
Phil would point a giantic looming finger down at Oprah and scream, "There comes A TIME ... when ya just have to say ENOUGH IS ENOUGH!" At which point Oprah would snap off his finger, chewing on it with great violence.
The audience would stand up and cheer on Oprah as Phil screamed. Phil would nurse his bloody finger as his wife Robin ripped out Oprah's hair. Then Oprah would slash Robin with her nails and pull her across her lap, spanking her wildly.
Then Phil's son (what's his name again?) would tear Robin off Oprah and offer his dad a tissue for his tears and some free psychological counseling.
Fade to another break ...
This would be a fight where everyone loses and everyone needs therapy. Good for ratings.

Oprah! She has been around longer than Dr. Phil and Im sure she can be a witch when she wants to be. Plus, make a woman angry and she will fight till the end, using fingernails, legs, arms..etc...
Dr. Phil is too respectful of women, he wouldn't hit her back. He would try to talk to her about using self control while she is slapping his face..lol.

Oprah will beat the breaks off Dr.Phil. I think since she lost the weight, and she knows self-defense, and she can move pretty quickly now (show with her and Kristy Alley dancing/working out). Phil will get tired to fast cuz i don't see him doing any mile runs or even talking about doing it. I would love to see that on Celebrity Death Match! I love Oprah man...LOL & Be Good...

Oprah would win but the Doctor would put up a really good fight first he would make her feel really self concious then he would play mind games with her and Oprah would get fed up with it and take out his heart.

I'd have to say Dr. Phil, he's big. I mean like 6'4" big. Oprah is big too, but only horizontally, and not tall enough to get to Dr. Phil. She might try to gnaw his leg off though.

in a fist fight or show of brute strength phil might have a
chance. in any other way oprah would just have to sit
back and wait until he blows himself out then take the
contest. phil is physically large, but in everything else
he is just a loud overbearing blowhard.
