I want to 'opt out' of the new NHS 'spine' which holds personal patient data. How do I 'opt out'?

I would like to opt out of the NHS spine so that my personal details are not available to about 50,000 NHS staff all over the UK. I have not been asked for my consent for these details to transferred to the spine and I would like them removed. However, my GP surgery does not have any information on how patients can 'opt out'.
