Assume that he had Alziemer's Disease and had wondered off from his home ar a retirement home and got lost.
They forget who they are, where they and do things like take off their clothes, simply because they have forgotten that, it is inapropriate to walk around naked.
I would assume the nursing home and or his family were frantic and looking for him.
People with this disease, frequently, wonder off and get lost. Their behavior is reduced back to when they were just a toddler.
I would call the police and an ambulance, via 911.
Being that I'm an RN, I would alert the police to his probable condition and have them check their missing persons reports to locate where he resides.

I would assume that he is crazy and stripped down. Took some acid and popped out his own eyeball to let the blood run across his wrinkled skin. Let him be... Let him be.

Call for medical assistance. Stay with him {Without touching him} until the ambulance arrives.
