Yes, it was done using an application I often use, called Adobe Photoshop, which is very, very good for doctoring up photos.
You can get very creative with it, like this artist did!

Continuity: When Jack is running inside the water mill wheel, he hits his head on a metal bar and falls out. When he catches up and starts running on the inside again, the bar is gone.
Crew or equipment visible: When Jack Sparrow falls into the grave, the print of a modern shoe can be seen clearly in the dirt at the top of the grave.

One odd thing from "Dead Man's Chest" - if the curse of the Aztec gold was lifted in the first movie, then why is the monkey still undead? (And how the hell will they explain Barbosa's reappearance?)

it looks kinda fake to me. i guess it could be real though. like in lord of the rings two towers when saurmon is staning on top of his tower and his hand is clearly all bandaged up. that stuff is funny. well, it's not funny he hurt his hand... but you know.
