They all leave though Celeborn is said to have stayed the longest. I cannot remember if Arwen left or not. I think there was some ambiguity in the appendix, a little left to the readers imagination as to whether or not she stayed on. I may be wrong on that though.

At the end of the Lord of the Ring: The Return of the King, these are the ones who sailed from the Grey Havens west to the Undying Lands, the Uttermost West.
Elrond, Galadriel, Gandalf the White, Bilbo, and Frodo. These are the Ring Bearer. With them Gildor Inglorion and many other Elves besides, but whether any of these others went aboard the ship with them is not told. Also the only horse who went was Shadow Fax.
Eventually, the High Elves but not the sons of Elrond (Elladan, Elrohir) and Arwen his daughter. The Green and Grey (Dark) Elves stayed and died with mortal men.
Some say Legolas and Gimli as sailed west.
The Encyclopedia of Arda is a great resource for all this stuff.
This required a lot of research. I hope I get the best answer for this one.

Eventually, the High Elves leave, along with Frodo, Bilbo and Gmili, as others have noted.
Those few High Elves that do not leave (such as the sons of Elrond) and the Green Elves and Grey Elves dwindle away in the Age of Men.

Only Arwen Undomiel daugter of Elrond, king of elves married with Aragorn, king of Gondor and stayed. But she Became Human since his father was human who married princess of elves and got a choice for the children that if they do not return with him they will become human.
Lagolas left with gimli. Three ring-bearer elves left with gandalf, frodo and bilbo.

Arwen didn't leave. she married aragorn, and stayed in Gondor with him. The others, I don't know about. I think the Keebler Elves stayed to provide Middle Earth with their tasty bakery goods.

Some of them stick around for a while but the do all leave eventually. Except, of course, for Arwen. But she became mortal so she doesn't really count because she ends up dying.
