The question is why does she choose to do this? Does she live alone and have no one in the world? In this case, she may suffer from depression.
Is she a busy single parent with lots of children to raise? If so then she doesn't have the time or energy.
Does she have a sense of smell? Most people can smell themselves when they haven't bathed, they just can't tell how bad it is.
It doesn't seem like she has a medical illness that causes the body to smell because you say she doesn't change her clothes.
How is her home or apartment? Is there a smell when you go close? Is it in disarray?
Is this person old or senile or overly intellectual?
I have experienced this situation numerous times and twice I told the people that they smell. Both times the fellows used no anti-perspirant. My personal choice is to tell the person directly in situations where it does them more harm than good.
I think addressing it may be fruitless if her house is in the same shape as her body as she probably has a mental illness and what she really needs in this case is professional help. If her house is perfectly fine, then I think she may be more responsive to your comments.

Send her a care package anonymously. Put in a tooth brush, toothpaste, shower gel, shampoo and a wash cloth. Put a book in there that talks about the importance of hygiene, or write a note saying "Please use these items daily" Or get someone to tell her out right that she needs to take care of herself better.

Avoid physical contact with her. The more you ignore her physically, the more she'll realize that something is wrong. Once she comes to you and asks, that is when you should tell her. Tell her nicely and choose your words very carefully. It doesn't make any sense at all that a grown woman like her smells like that. She should be ashamed of herself! Ignore her for a while and see what happens.

I don't think that is possible, but it's a trade off- help her for her future or piss her off. I'd sit her down as a friend and tell her. you never know she may not know any better, or may have a situation at home that she can't. Maybe you could help her...
Either way it may embarrass her a little but at least she will be clean

I can send an anonomous note if you have her address. we did this and had a friend from california send it to someone.she wouldnt have any idea who did it. anyway, the person started to improve. but it begins with the parents training. i train my daughter to keep her self clean and some of her friends are dirty and then i see the mother!
