If you are not sure what to buy, the best thing to do is give a plush toy with a gift certificate from a baby store, so the mother can use to by something for the baby when the need arises, or cash.

diapers diapers diapers, coming from a new mom, the diapers are what I needed the most. Blankets and toys I have thousands of and your friend probably has already gotten them too. If she's formula feeding you could ask her what kind and get her some of that too. But pampers swaddlers are the best diapers so show that little boy some lovin' and get him some of those! Your friend will really be grateful!

Depends on the overall situation.
If money is an issue, consumables are a great idea---diapers, diaper wipes, infant Tylenol or Motrin, infant Pediacare, baby bath, lotion, diaper rash stuff, etc.
Other ideas:
The baby isn't going to know you gave him a present. Especially when I know the parents aren't hurting for money and had umpteen baby showers, I give them something for them---a gift basket with a bottle of champagne, some chocolate or snacks, and a movie.
I live in DC, and when these people I know who are fairly well off had their first child, I went and bought team apparel for the Redskins, Wizards, Capitals and Orioles (this was pre-Nationals).

A new born needs clothes......something very cute to show off in, as they wont be wearing it very long, A blanket perhaps yes, but I would suggest going to a nice upper class type baby store and finding something really original and something they will keepsake perhaps...
Toys really un-needed as a newborn...unless its something they can play with later, stuffed animals great for decoration and drool catching little else...lol

The book Mike Mulligan and His Steam Shovel.
Every boy, no matter how small, should have a copy of that book!
Who cares if he is too small to appreciate the story right now, he still loves to hear the voices of his parents and knows the pleasure of being held by them. (This holds true for his parents as well.)
All of us can certainly use the sense of being adored.
I must add...I did the same thing with my daughters. I just chose different books for them.

Diapers, baby wipes, bath soap, lotion, nail clippers, socks....I have 2 kids and these are all of the things that I ran out of most often! Make a basket with all of these things in it, that will be a great gift and will help the parents considerably!

Diapers!!! I have a 2 yr old boy, and the best thing I got was diapers. Babies grow fast, so get her some size 2 or 3 so she has them when she needs them.

The best gift you can get them is DIAPERS. Something every new mom can use and maybe a cute little outfit on the side. :) There is nothing more adorable than a precious little newborn. Aww... congrats to your friend!

My son is 4 months old and he is growing everyday. My suggestion would be snap shirts. They are easy to change for spitups and also help when the cord is falling off. Plus, easy access to changing diapers fast, since sometimes they squirt everywhere. Congrats to your friend, having a baby is wonderful!
