If you take a reed a peel it apart, a powder comes out of certain reeds. This is a good tooth cleaner, and you would brush with the reed. There was also hardly any sugar eaten, so you wouldn't have to brush nearly as much. For washing, you would go to the river and scrub sand on yourself, it gets out most of the grime.

they just bathed in water with a scrubbing brush, most people could not even take a bath, most old towns used to have a community bath (really it is was a horse troth filled with water) and the townspeople would get one bath per month! oh the little things we take for granted

They used ashes to brush their teeth with, and similarly to MAKE soap. The ashes are put in a pot and boiled and the top is skimmed (which is lye), periodically, and placed in another pot with a bit of wax. It makes soap. Often, they would scent it with crushed flower petals. My grandmother still makes lye soap this way.

soap has pretty much always been around, it was made out of animal fat and bone before they made modern day soaps.

as for brushing the teeth, toothpaste generally wasn't needed like it is today. think of the food we eat! think of how fake and bad most food is for your teeth today. natural "toothbrushes" like apples and good vegetables were used.
