There isn't any first, second, or third world countries; those terms were used during the Cold War to describe U.S. and U.S. allies, the U.S.S.R. and all countries below the U.S.S.R. Anyway, it's now developed and developing nations. There are many measures to determine if a nation is developed; it depends a lot on the Human Developmental Index, government centralization, industrial base, infrastructure, GDP, literacy rates, etc.
Africa provides the best example of developing nations; it is by far the most under-developed continent. They really need the support of the rest of the world right now.

No, but up until recently it hadn't been an issue. Now some of the developing world nations (which is now the preferred terminology) are rising almost to the level of the developed world, so everyone is uncertain how to classify South Korea or Taiwan or Singapore, for example. Even Chile is close to reaching that mid-status.
As far as 2nd world goes, as this was all Cold War terminology, the 2nd world referred to the Soviet Union and satellites, so there's little left of that today (Cuba, Vietnam, Laos, maybe Byelorus and Turkmenistan, and China, I guess - the remnants who never quite got around to throwing off the Communist dictators), but we never really referred to the 2nd world countries by that name anyway. There was never a "4th world" in that outline.

No. That is all left over from the Cold War. The term third world is a term used to describe the countries that were not aligned with the US and were not aligned with the USSR.
