How do I designate her as his caregiver for the week when we are not going to be there in case of an accident? So she can use our insurance

I believe you just write a letter and have it noterized and she carries it with her just in case. Be sure your phone number is on there so if anyone has questions they can contact you.

Simply write out your authorization or type it out on the computer, print it for the grandmother to have in her possession. List your contact numbers, dr. numbers, you might even copy your medical ins. card or give her yours. In cases of emergency with minors you really only need to provide I.D. as gaurdian.

It sounds like you are talking about an Emergency Authorization. It would be similar to what you sign for daycare. A form stating that you give grandma permission to seek medical treatment for your child.

A letter with all of your contact information and sign it. It can be notarized or not depending on you state laws.
