Books: Usage and Abusage by Eric Partridge.
Fowler's Modern English Usage.
First is my preferred choice.
Online version of the latter still exists, I think.

all the entry exams as well as TOEFL GMAT CAT etc have lots of grammar exercises with resloved /answers in them. first figure out where u are weak is it tenses, parts of speech, etc and concentrate on that area. Old TOEFL books like 20 yrs old are really good.
