Dear Sir/ Madam,

Due to your incompetence and inept abilities to teach, I hereby formally put forth to you that you are not up to the mark to be my teacher and should not be in anyway still be in this field which you may in future cause many other potential high-flyers to be mislead by your incapabilities and thus have their future ruined.
I implore upon you as a friend and student that you give up when you are still remembered as a teacher.

I wouldnt write the letter, unless you are a co signatory and it comes from the entire class. You need to think about what the consequences would be from such a letter and what benefit or damage could happen to you

Instead of criticising someone who is quallified to teach you because you cant learn, think about what YOU do and what YOU can do to help YOURSELF...ever heard of the saying "You can lead a horse to water..."???
